UNIVERSAL CASE MANAGEMENT (UCM)
About UCM
The role of Universal Case Management (UCM) will be to advocate for and coordinate services and supports for individuals and their families. The focus is on the individual and support preferences and choices for meaningful lives within a context that promotes dignity and the protection of their individual rights.

The Universal case manager is a bridge between the individual and their family with service providers, funding agencies as well as other professionals. In order to accomplish our goals the Universal Case Managers will:

  • Organize and participate in the intake of new referrals as well as transition and discharge planning.
  • Organize, schedule and facilitate the Individual Plan meeting.
  • Insure coordination of all services and supports through communication with individuals and their families as well as service providers and the Division of Developmental Disabilities (DDD).
  • Meet regularly with the individual at work and at home as well as with their families.
  • Serve as a liaison with the individual, family, DDD, service provider for consumer/family needs, issues or problems and maintain ongoing communication with all members of the support team in a timely and effective manner.


What does this mean to me as a parent?
Individuals and their families will have ongoing communication with the Case Manager and will be able to call when needed.

They will be able to call the Case Manager to get information about all services currently received. Case Managers will be advocates for the individual and their families and will work on their behalf.

Individual Plan (IP)
The Individual Plan is an annual process designed to assist individuals and those persons closest to them to develop a vision and to attain positive outcomes in critical life areas while promoting individual control, choice and self-determination. Families are an important part of this process and we encourage your participation.
